Rapid Housing Response

Housing crises do not keep office hours. Our rapid response pathway pairs an available accommodation with a vetted support team so participants can move out of unsafe or unsuitable environments quickly.

We coordinate directly with the NDIA, support coordinators, hospitals and emergency services to make sure the transition is documented, funded and clinically safe.

We triage on the same business day and most participants are onboarded within one to two weeks. Rapid response pathways are faster.

Do you work with plan-managed and self-managed participants?▾

Yes. We work with agency, plan-managed and self-managed participants.

How is reporting handled?▾

We provide written reports to the participant, nominee and support coordinator on an agreed cadence — at minimum monthly.

Can this be combined with your other services?▾
