Hospital Discharge

A safe discharge is more than a bed. Our team meets with hospital social workers, allied health and treating teams to align supports, equipment, medication and routines before the participant leaves the ward.

We provide accommodation, rostered support and follow up coordination so participants are not readmitted because the community handover was rushed.

Yes. We work with agency, plan-managed and self-managed participants.

We provide written reports to the participant, nominee and support coordinator on an agreed cadence — at minimum monthly.
